How do I know if I'm in perimenopause?
Perimenopause is diagnosed mainly from your symptoms and cycle changes, not from a single blood test. The clearest early sign is a change in your periods: cycles that get shorter, longer, heavier, lighter, or begin to skip, usually starting in the early-to-mid 40s for Indian women. Alongside this, many women notice hot flashes, disturbed sleep, mood swings, brain fog, or new PMS-like symptoms. If you are in your 40s and your cycle pattern has changed together with any of these, you are most likely in perimenopause. A doctor confirms it from your history and pattern, sometimes using tests to rule out thyroid problems or anaemia rather than to diagnose perimenopause itself.
Is there a blood test for perimenopause?
There is no single reliable blood test that confirms perimenopause. Hormone levels, especially FSH and oestrogen, swing widely from day to day during the transition, so one reading can look normal even when your symptoms are clear. This is why NICE guidance recommends diagnosing perimenopause in women over 45 from symptoms and cycle changes alone, without routine hormone testing. Blood tests are genuinely useful in specific situations: to check the thyroid (thyroid problems are common in Indian women and easily mistaken for perimenopause), to check for anaemia when periods are heavy, or when symptoms begin before 40 and early menopause needs ruling out.
When does perimenopause start and how long does it last?
Perimenopause usually begins in the early-to-mid 40s, though it can start earlier for some women. Indian women tend to reach menopause a little earlier than Western averages, around 46 to 48 years. The transition itself lasts about 4 years on average, but the range is wide: some women pass through it in a year or two, others take 8 to 10 years. It formally ends 12 months after your final period, which is the point at which menopause is reached.
What are the first signs of perimenopause?
The earliest sign is almost always a change in your menstrual cycle: periods that become irregular, shorter or longer, heavier or lighter, or start to skip. Other early signs include hot flashes and night sweats, disturbed sleep, mood changes or irritability, brain fog, worsening PMS, new joint aches, and changes in libido. They tend to arrive gradually and vary a great deal from woman to woman; you may notice only a couple of them or several at once. None of this means something is wrong with you: it is your body moving through a normal transition, and most of these symptoms respond well to the right support.
Can you get pregnant during perimenopause?
Yes. As long as you are still having periods, even irregular ones, your ovaries can still release an egg, so pregnancy remains possible throughout perimenopause. Fertility is lower than in your 20s and 30s, but it is not zero. If you do not wish to conceive, you still need contraception until you have gone 12 months without a period (or 24 months if you are under 50). If you are hoping to conceive in your 40s, it is worth speaking to a doctor early, since the fertile window is narrower.

Perimenopause is the transition leading up to menopause: the years when hormones fluctuate, periods become irregular, and symptoms like hot flashes and disturbed sleep begin, while you are still having periods. Menopause is a single point in time, the day you have gone 12 full months without a period. Everything after that is called post-menopause.
